PROGRAMS WE OFFER

The Fresno Center's Monthly Food Distribution

The Fresno Center’s Monthly Food Distribution program addresses food insecurity in Fresno County by providing individuals and families with food boxes containing ingredients procured from local farmers, food suppliers, and the Central Community Food Bank.

Team and Changes

Assistance & language support to clients with natural gas and electricity energy (PG&E) as well as phone assistance. We will help concern clients related with disputing their utility bills by reducing or adjusting their cost.

Stop the Hate (STH)

We are here to help create a more inclusive local and regional alliance and offer support for those who are historically vulnerable because of their racial, religious, gender/gender expression, sexual orientation, or other identities.

CalAIM Community Supports (CS)

Flexible, wrap-around services designed to address medical and socially determined health gaps. Eligible Medi-Cal members, regardless of their qualification for ECM services, can apply for assistance with housing transition and navigation services. These services aim to help individuals secure and maintain housing, whether they are chronically homeless, homeless, or at risk of becoming homeless.

CalAIM Enhanced Care Management (ECM)

A statewide Medi-Cal benefit available to eligible members with complex needs. Enrolled members receive comprehensive care management from a single lead care manager who coordinates all aspects of their health and health-related care, including physical, mental, dental care, and social services. ECM facilitates access to the right care at the right time and in the right setting, ensuring quality care that extends beyond the doctor's office or hospital.

Fresno HOPE (FHOPE)

Fresno HOPE (Health, Outreach, Prevention, Equity) coordinates agencies and community organizations to reduce health disparities in Fresno County by empowering Community Health Workers (CHWs) to provide culturally appropriate services, collect data, and enable families to participate in their health outcomes.

Trusted Messenger Network (TMN) Program

The Trusted Messenger Network (TMN) program raises awareness, mobilizes support, and provides accurate information on critical state issues to Fresno's most vulnerable, aiming to improve quality of life through inclusive outreach campaigns.

Cultural Based Access Navigation Support Services (CBANS)

The CBANS program operated by The Fresno Center uses Community Health Workers and Peer Support Specialists to provide culturally responsive navigation, improving access to behavioral health services for unserved and underserved populations by linking them with providers and systems of care.

Southeast Asian Economic Development and Community Coalition (SAEDCC)

The Southeast Asian Economic Development and Community Coalition (SAEDCC) empowers underserved Southeast Asian communities in Fresno County by providing access to essential resources, education, support services, and advocacy to foster economic growth, promote social equity, and enhance community well-being.

Central Valley Regional Center (CVRC)

CVRC provides assessments, evaluations, programs, activities, and case management for people who experience intellectual and development disabilities.
