EMPOWERING HEALTH
Enhanced Care Management (ECM) is a program funded under the California Advancing & Innovating Medi-Cal Initiative. The program is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of Members with the most complex medical and social needs. The ECM program provides systematic coordination of services and comprehensive care management that is community based, interdisciplinary, high tough and person centered.

Services provided under this program are covered by an individual’s Medi-Cal insurance (Anthem Blue Cross or CalViva/Health Net).
Eligibility:
Current Medi-Cal beneficiary
Resident in Fresno County
Meet the requirements for one of the Populations of Focus (PoF):
Adults with Serious Mental Illness (SMI) or Substance Use Disorder (SUD)
Children or Youth with Serious Emotional Disturbance (SED)
Children or Youth at clinical high risk of Psychosis or first episode of Psychosis
Children or Youth (up to 21 years old) involved in Child Welfare, California Children Services (CCS)/Whole child Model with additional needs

Health Promotion
Linkages and referrals
Member and family supports
Coordination of and referral to community and social services
