Enhanced Care Management (ECM) is a statewide Medi-Cal benefit designed to support eligible members with complex needs. With a dedicated lead care manager by your side, ECM coordinates every aspect of your health—physical, mental, and dental care, along with essential social services. More than just medical care, ECM ensures you get the right support at the right time, in the right place—so you can focus on living well beyond the doctor’s office or hospital.

Eligibility:
Current Medi-Cal beneficiary
Resident in Fresno County
Meet the requirements for one of the Populations of Focus (PoF):
Adults with Serious Mental Illness (SMI) or Substance Use Disorder (SUD)
Children or Youth with Serious Emotional Disturbance (SED)
Children or Youth at clinical high risk of Psychosis or first episode of Psychosis
Children or Youth (up to 21 years old) involved in Child Welfare, California Children Services (CCS)/Whole child Model with additional needs
